Your kidneys have many functions, some you may not have known. Your kidneys help to flush out waste products and excess fluids continuously. They also help to make new red blood cells that carry oxygen throughout your body. They also help maintain your bone health and balance vital nutrients such as Sodium, Potassium, and Phosphorous. As kidneys become damaged or when we don’t maintain a healthy diet and lifestyle, kidney function can decline. CKD can also be caused by inherited conditions, malformations and other medical conditions. With improved diet, lifestyle changes, and medications under the management of your physician, progression of CKD may be slowed.

Have You Been Diagnosed with CKD?

Chronic Kidney Disease (CKD) can be managed with proper diet and lifestyle changes. With a referral from your Nephrologist or primary care physician, Medical Nutrition Therapy for non-dialysis (CKD) is a covered service under Medicare Part B. Participants under this plan are allowed 3 hours of Medical Nutrition Therapy for the 1st calendar year, then 2 hours of Medical Nutrition Therapy each calendar year thereafter. As a Medicare provider, Horizon West Nutrition Services will provide counseling to help manage your current stage of CKD, offer meal planning assistance and provide ongoing monitoring and support as needed.
